Microneedling is a texture and quality treatment. It works gradually and cumulatively rather than dramatically in one visit.

Most resurfacing treatments work by heat — laser energy absorbed and converted to thermal injury. Heat is exactly what provokes pigment cells in Fitzpatrick IV-V skin, which is why ablative laser on brown skin needs careful protocols and careful timing.

Microneedling is purely mechanical. The collagen response happens without a thermal insult, so the risk of post-inflammatory hyperpigmentation is markedly lower. For patients who want real textural improvement but cannot accept the risk or the downtime of CO2 laser, this is usually the answer.

Patients ask us to choose between these constantly, so here is the honest comparison rather than a sales pitch for whichever is more expensive.

CO2 laser is stronger per session, delivers more improvement in fewer visits, and costs more in both money and downtime — around five to seven days visible recovery, and a real pigmentation risk to manage. Microneedling is gentler per session, needs four to six visits to reach a comparable place, has one to two days of downtime, and is considerably safer on deeper skin tones.

For deep established scarring, CO2 usually wins. For moderate scarring, pores and general texture — particularly on darker skin or when a week of downtime is impossible — microneedling is the better trade. Many patients do best with a combination, and we will say which applies to you at assessment.

The skin is cleansed and topical numbing cream is applied for about thirty minutes. The device is then passed over the treatment area at a depth chosen for the region — shallower around the eyes and forehead, deeper over scarred cheeks. There is pinpoint bleeding in the deeper areas, which is expected and settles within minutes.

A serum is applied afterwards, commonly hyaluronic acid or PRP drawn from your own blood, which absorbs far more effectively through the fresh channels than it ever would through intact skin. You leave looking as though you have moderate sunburn.

Day one is red and warm, comparable to a strong sunburn. Day two is pink and often slightly rough or tight. By day three most patients are back to normal, sometimes with light flaking.

Makeup after twenty-four hours, no actives for about five days, no gym, sauna or swimming for forty-eight hours, and daily sunscreen throughout. It is a treatment that fits around a working week, which is a large part of its appeal.

Home rollers are sold everywhere and we see the results. The needles blunt quickly and then tear rather than puncture, sterility is impossible to maintain in a bathroom, depth cannot be controlled or varied by region, and rolling over active acne spreads bacteria across the face.

The clinical version uses a sterile single-use cartridge, a vertical stamping motion rather than a drag, controlled and adjustable depth, and a trained operator who knows which areas to avoid. It is a genuinely different procedure that happens to share a name.
