Filler is a structural treatment. Used well it restores proportion; used badly it distorts it.

Hyaluronic acid filler can be dissolved. If the result is not what you wanted, if it migrates, if it is placed asymmetrically, or if there is a vascular complication, an enzyme injection reverses it. That single property is why it is the responsible choice.

Permanent and semi-permanent fillers — silicone and some other materials still injected in this market — cannot be removed without surgery. When they go wrong, and over a long enough timeframe a proportion do, the patient is left with a surgical problem. We do not use them and we would advise against anyone who offers them.

Filler is a medical procedure and carries real risks. Common and expected: swelling, bruising, tenderness, temporary lumpiness that settles. Uncommon: infection, prolonged nodules, asymmetry needing correction.

The serious one is vascular occlusion — filler entering or compressing a blood vessel, cutting off blood supply to the tissue it feeds. It is rare, it is a genuine emergency, and it can cause skin loss or, in the worst reported cases involving the area around the eyes and nose, visual damage. It is managed by an injector who recognises it immediately and has dissolving agent to hand.

This is precisely why the question of who is injecting matters more than the price. Ask any clinic what they would do if this happened. If the answer is vague, leave.

Lip filler is the most requested and the most over-done treatment in aesthetics. The results that look obviously "done" are almost always the result of too much product, too fast, or product placed to chase a shape the patient’s anatomy does not support.

Our approach is to start with a conservative amount, let it settle for two weeks, and build at a second appointment if you want more. Most patients stop after the first. We will also decline to reproduce a photograph of somebody whose lip anatomy is fundamentally different from yours, and explain why.

Tear-trough filler transforms the right candidate and looks terrible on the wrong one. The skin there is the thinnest on the body, the anatomy is unforgiving, and the complication rate is the highest of any common filler site.

Poor candidates include patients with significant eye-bag fat herniation, thin crepey skin, or a tendency to fluid retention — in all of these, filler puffs, appears bluish through the skin, or migrates. If that is you, we will say so and discuss alternatives rather than take the booking.

Expect swelling. Lips swell dramatically for the first twenty-four to forty-eight hours and look nothing like the final result — do not panic on day one and do not book a wedding for day three. Bruising is common and settles over three to seven days.

Avoid exercise, alcohol, heat, saunas and facials for forty-eight hours, sleep slightly elevated, and do not massage the area unless instructed. Judge your result at two weeks.
