Botox works on dynamic lines — the ones caused by muscle movement, which appear or deepen when you make an expression. It does not work on static lines etched into the skin at rest, and it does not replace lost volume.

The frozen, shiny, immobile look people fear is not what Botox does — it is what too much Botox in the wrong places does. Our default is to start below the dose we think you need, review you at two weeks, and add a little if required.

This costs us a second appointment and it is the right way round. Botox that has been under-dosed can be topped up in ten minutes. Botox that has been over-dosed cannot be removed, and you live with a dropped brow or a blank forehead for three months. Any injector who does not offer a two-week review is not planning to correct anything.

We decline treatment more often than patients expect, and the reasons are worth stating plainly.

You will be asked to make a series of expressions so the doctor can map how your muscles actually move, which varies considerably between faces. Injection points are marked, and treatment takes a few minutes using very fine needles. Most patients describe a brief sting.

You will have small raised marks for around thirty to sixty minutes, which settle. Occasional small bruises are possible. Avoid lying down flat, exercising, or massaging the area for the rest of the day.

Nothing happens for the first three days. Movement begins to soften around days three to five and reaches full effect at about two weeks, which is when the result should be judged rather than on day four.

Effect typically lasts three to four months, less in people with strong muscles or high metabolic rates, sometimes longer after several consistent treatment cycles. If you stop, the muscle gradually returns to its previous activity and lines return to where they would have been. Botox does not make things worse when discontinued, which is a common and unfounded fear.

Botulinum toxin is expensive, which creates a market for counterfeit and improperly stored product, and Pakistan has one. Diluted, fake or heat-damaged toxin produces a weak result at best and an unpredictable one at worst.

You are entitled to ask any clinic what product they are using, to see that the vial is opened in front of you, and to know who is injecting it and what their qualification is. Ask us. Ask everyone. A price that is dramatically below the market is not a bargain in this category — it is a signal.
