Exosomes are tiny vesicles that cells release to signal to one another, carrying proteins and genetic messages that influence how the receiving cell behaves. What exosomes are, without the marketing

Every cell in your body releases exosomes. They are how cells send instructions to their neighbours — a package of growth factors, proteins and RNA that tells the receiving cell to do something. In regenerative medicine the interest is in harvesting concentrated exosomes and using them to deliver a strong growth signal to tissue that has become quiescent.

Applied to hair, the target is the miniaturising follicle: not dead, but producing progressively finer hair. The intention is to push it back into a proper growth phase. This is the same target PRP addresses; the difference is the delivery vehicle and the concentration of signal.

Who it suits

The candidacy rules are close to those for PRP, and the same honest limit applies: a follicle that is gone cannot be signalled back.

  • Good candidates: early to moderate thinning, increased shedding, diffuse thinning along the parting, patients who have plateaued on PRP
  • Also useful: supporting graft survival and surrounding native hair after a transplant
  • Poor candidates: areas bald for several years with no follicular activity remaining
  • Not appropriate: undiagnosed hair loss, which needs investigation first — thyroid, iron, autoimmune causes

How it compares to PRP, honestly

This is the question every patient asks and it deserves a straight answer rather than a price-led one.

PRP uses your own blood, has a longer clinical track record, costs less per session, and requires a blood draw and processing time. Exosome therapy uses an external laboratory-produced preparation, requires no blood draw, delivers a more concentrated and consistent signal, costs more, and has less long-term safety and efficacy data behind it.

In our experience exosome therapy tends to show a faster reduction in shedding, and some patients who have stopped responding to PRP do respond to it. What we will not claim is that it is definitively better, because the head-to-head trials that would establish that have not been done at the scale required. Patients are often best served by a combination, and we will tell you if we think PRP alone is sufficient for your situation.

What a session involves

The scalp is cleaned and numbing is applied where needed. The preparation is injected across the affected areas in a fine grid, sometimes combined with microneedling to aid distribution. It takes around thirty to forty-five minutes and there is no blood draw.

Your scalp will be tender for about a day with pinpoint marks that settle. Wash normally after twenty-four hours. Avoid the gym, sauna and swimming for the rest of the day, and that is all the aftercare required.

What to expect, and how to actually measure it

The first change is a reduction in shedding, usually at four to eight weeks. Because this is a change in something you were losing rather than something appearing, it is very easy to miss.

We take standardised baseline photographs before starting and repeat them at the same angles and lighting through the course. Hair changes too gradually to judge from memory, and patients routinely under-estimate their own improvement when comparing against a mental image rather than a photograph. If a clinic does not photograph you at baseline, neither of you will be able to tell whether it worked.

Why medical therapy still matters

As with PRP, exosome therapy improves the hair you have. It does not switch off the hormonal process driving pattern loss. Used alone in progressive androgenetic alopecia, it is swimming against a current that keeps flowing.

The plans that hold combine regenerative treatment with medical therapy to slow the underlying loss, plus correction of anything treatable found on investigation. That is a less exciting sentence than most of the marketing in this category, and it is what actually works.

What affects the cost

Exosome therapy is the most expensive of the injectable hair treatments and is priced per session. What changes the cost:

  • Number of sessions in your course, typically two to four
  • The preparation used and its concentration
  • Area treated — hairline, crown, or full scalp
  • Whether it is combined with PRP or microneedling in the same session
  • Whether it is being used to support a hair transplant
  • Loyalty membership, across a multi-session course

We will tell you at assessment if we think PRP alone would serve you just as well, because for a proportion of patients it would. Being sold the most expensive option by default is how this category loses people’s trust.

Is exosome therapy better than PRP?

There is no reliable head-to-head evidence establishing that, despite how it is marketed. Exosome therapy delivers a more concentrated and consistent signal and often reduces shedding faster, but PRP has a longer track record and costs less. Some patients who plateau on PRP respond to exosomes. Many do well on a combination.

How many sessions do I need?

Typically two to four sessions spaced four to six weeks apart, then maintenance depending on your response. Fewer sessions than a PRP course, at a higher cost per session.

When will I see results?

Reduced shedding at four to eight weeks, visible thickening at three to six months. We take baseline photographs at the start because gradual hair change is genuinely difficult to judge without them.

Will it regrow hair on a bald area?

No. Exosome therapy signals to follicles that are still present but miniaturising. Where the follicle is gone — typically scalp that has been completely bald for several years — there is nothing to signal to. That area needs a transplant, not injections.

Is it safe?

The reported safety profile is good, with side effects limited to temporary scalp tenderness, minor swelling and pinpoint marks. It is a newer treatment than PRP, so the long-term data set is smaller. We discuss that openly rather than leaving it out.

Can I have it after a hair transplant?

Yes, and it is a common reason to use it. Regenerative treatment after a transplant aims to support graft survival and to help the native hair around the grafts, which is the hair most at risk of continuing to thin.
