CO2 fractional laser is a texture treatment. Where the problem is the physical surface of the skin, it is the strongest tool available. Where the problem is colour, it is usually the wrong tool.

Melasma is the important one. CO2 laser generates heat, heat is a melasma trigger, and treating melasma with an ablative laser frequently makes it substantially worse. If you have melasma and textural scarring together, the melasma is stabilised first and the laser settings are adjusted around it.

It also does not replace volume. Deep folds and sagging need fillers, threads or surgery, not resurfacing. And it does not treat active acne — inflamed skin should not be lasered.

Here is what actually happens, day by day, so you can plan rather than panic.

Ablative laser on Fitzpatrick IV-V skin carries a real risk of post-inflammatory hyperpigmentation. This is not a reason to avoid the treatment; it is a reason to be treated by somebody who adjusts for it.

In practice that means priming the skin with pigment-suppressing topicals for two to four weeks beforehand, using conservative density and energy rather than maximum settings, spacing sessions six to eight weeks apart, and being rigid about sun protection during recovery. We also prefer to schedule CO2 work between October and March. A clinic willing to run full-strength CO2 on brown skin in June is a clinic to walk out of.

Preparation starts two to four weeks before: pigment-suppressing topicals if prescribed, stopping retinoids a few days prior, no sun exposure or tanning, and antiviral cover if you are prone to cold sores, because laser can reactivate them.

Aftercare is simple and non-negotiable. Keep the skin barrier occluded and moist, cleanse gently with lukewarm water, do not pick or exfoliate the flaking skin, avoid the gym, sauna and swimming until healed, and apply broad-spectrum sunscreen religiously from the moment the skin tolerates it. Almost every bad CO2 outcome we are asked to fix traces back to picking or to sun exposure during recovery.

For general texture and fine lines, one well-executed session often delivers most of the result, with a second optional. For established acne scarring, plan on two to three sessions spaced six to eight weeks apart, frequently combined with subcision for tethered scars.
